Each of these forms is to be filled out and returned if you haven't already. Band fees are due by September 30th.

Physicals
Every participating member of the Lion Pride Marching Band is required by GISD and UIL to have a physical on file at the start of their freshmen (9th grade) and junior (11th grade) years before they can participate in any UIL sanctioned activities (ex: Football Games, Marching Contests, etc.)
If you DO NOT have a physical on file with GISD, you will need to complete the GISD Physical Packet as well as the UIL Pre-Physical Evaluation.
If you DO have a physical on file with GISD, you will ONLY need to complete the first page of each form.
